To establish service with Fort Payne Water you will need TWO FORMS OF IDENTIFICATION (GOVERNMENT ISSUED) with at least ONE being from the Primary Forms List.

Primary Forms:
  • - Valid Driver's License
  • - Non-Driver's ID
  • - US Passport
  • - Gun Permit w/Photo
  • - US Military ID Photo
  • - FAA Issued Pilot License
  • - Matricula Consular ID Card or similar forms of ID issued by other national governments
Secondary Forms:
  • - Social Security Card
  • - Birth Certificate (certified copy)
  • - Voter's Registration
  • - Medicaid or Medicare Card
  • - W-2 Forms
  • - Individual Tax Identification Number
  • - Hunting, Fishing, or Gun Permit w/o Photo
  • - Selective Service Identification Card
  • - Naturalization Document
  • - Employee Identification Card
  • - COVID-19 Vaccine Card
